Tax Return Release Form

This form allows someone other than yourself to pick up your tax return on your behalf.

Please do the following steps to complete our Tax Return Release Form:

  1. Click on the "View Tax Return Release Form" button below to bring up the .pdf file in a new window.

  2. Print the form by clicking on the printer icon at the top of the screen.

  3. Fill out all the information, and bring in/fax/email all supporting/tax documents.
